Telstra is on the search for its fifth chief information officer in six years as the carrier confirmed Erez Yarkoni would step down in October.
A Telstra spokesperson confirmed to CRN that Yarkoni would depart after 18 months in the role, having been named CIO in February 2015.
"After developing and leading the refreshed IT strategy, and realigning the IT operating model Erez has made the decision to move permanently back to the US to join his family.
"He departs Telstra with a clear road map for us to execute and deliver on our vision to become a world-class technology provider.
The spokesperson confirmed that Erez would remain in the role until October 2016 "to ensure a smooth transition of his leadership. A replacement will be announced closer to this time."
Yarkoni was promoted to the CIO after leading Telstra's cloud business.
His departure comes less than two weeks after the carrier revealed the departure of chief operating officer Kate McKenzie.
In July, Telstra's channel chief, director of business and enterprise partners Keith Masterton, left the telco after 26 years.
Telstra's CTO Vish Nandlall departed amid a storm of controversy in May.
In March, Telstra hired former high-profile Microsoft and Nokia veteran Stephen Elop to the newly created role of group executive technology, innovation and strategy.
